Our team of experienced therapists will thoroughly evaluate all body regions, from the area of pain to all contributory areas, to assess the true source of the issue. Book a one-hour in-depth assessment with one of our orthopedic sports medicine physical therapists. To ensure that your Physical Therapy (PT) evaluation is covered by your insurance, please submit a copy of your insurance card and date of birth, along with a physician’s referral. This will help verify if both your evaluation and treatment are covered by your insurance plan. Please provide the required information at least 72 hours before your appointment to allow sufficient time for verification and to ensure a smooth scheduling process.
